Please sign the form below to register for the MapúaWorld 2011 Banff and Lake Louise Day Trip. Registration fee is US$115 per person. The registration fee does NOT include Lunch. Transportation is included to the price, meeting place is at Hyatt Hotel.
Your payment may be remitted as follows:
A) Through PayPal (please follow procedure payment below)
1) Click on the "Donate" button. The MapuaWorld 2011 Paypal Account will open up.
2) Type $115.00 at the "Donation Amount" text box then click "Update Total".
3) Log-in to your PayPal account... this executes the transaction.
* The same procedure applies if you do not have a PayPal account and wish to pay by credit card or bank account.
4) Click on "Add special instruction to the seller" and type Banff and the names of persons you're paying for.
